Improving the lives of kids with disabilities – one family dog at a time

The family dog could serve as a partner and ally in efforts to help children with disabilities incorporate more physical activity into their daily lives, a new study from Oregon State University indicates.

In a case study of one 10-year-old boy with cerebral palsy and his family’s dog, researchers found the intervention program led to a wide range of improvements for the child, including physical activity as well as motor skills, quality of life and human-animal interactions.

“These initial findings indicate that we can improve the quality of life for children with disabilities, and we can get them to be more active,” said Megan MacDonald, an assistant professor in OSU’s College of Public Health and Human Sciences and corresponding author on the study. “And in this case, both are happening simultaneously, which is fantastic.”

The researchers detailed the child’s experience in the adapted physical activity intervention program in a case study just published in the journal Animals.

Children with physical disabilities such as cerebral palsy spend significantly less time participating in physical activity compared to their peers and are considered a health disparity group, meaning they generally face more health concerns than their peers.

Researchers designed an adapted physical activity, animal-assisted intervention where the family dog would serve as a partner with the child in physical activities designed to help improve overall physical activity, motor skills and quality of life. The family dog is a good choice for this type of intervention because the animal is already known to the child and there is an existing relationship – and both the dog and the child will benefit from the activities, MacDonald said.

Researchers took initial assessments of the child’s daily physical activity, motor skills and quality of life before starting the eight-week intervention. A veterinarian examined the dog’s fitness for participation and the human-animal interaction between the dog, a year-old Pomeranian, and the child was also assessed.

Then the pair began the eight-week intervention, which included a supervised physical activity program once a week for 60 minutes and participation in activities such as brushing the dog with each hand; playing fetch and alternating hands; balancing on a wobble board; and marching on a balancing disc.

“The dog would also balance on the wobble board, so it became a challenge for the child – if the dog can do it, I can, too,” MacDonald said. “It was so cool to see the relationship between the child and the dog evolve over time. They develop a partnership and the activities become more fun and challenging for the child. It becomes, in part, about the dog and the responsibility of taking care of it.”

The dog and the child also had “homework,” which included brushing the dog, playing fetch and going on daily walks. The child wore an accelerometer to measure physical activity levels at home.

At the conclusion of the intervention, researchers re-assessed and found that the child’s quality of life had increased significantly in several areas, including emotional, social and physical health, as assessed by the child as well as the parent. In addition, the child’s sedentary behavior decreased and time spent on moderate to vigorous activity increased dramatically.

“The findings so far are very encouraging,” MacDonald said. “There’s a chance down the road we could be encouraging families to adopt a dog for the public health benefits. How cool would that be?” 

The researchers also found that the relationship between the dog and the child improved over the course of the therapy as they worked together on various tasks. The dog’s prosocial, or positive, behavior toward the child is a sign of wellbeing for both members of the team, said Udell, who is director of the Human-Animal Interaction Lab at OSU.

“A closer child-dog bond increases the likelihood of lasting emotional benefits and may also facilitate long-term joint activity at home, such as taking walks, simply because it is enjoyable for all involved,” she said.

This study is one of the first to evaluate how a dog’s behavior and wellbeing are affected by their participation in animal-assisted therapy, Udell noted. From an animal welfare standpoint, it is promising that the dog’s behavior and performance on cognitive and physical tasks improved alongside the child’s.

Though the case study features only one child, the research team recruited several families with children with disabilities and their dogs to participate in the larger project, which was designed in part to test the design and methodology of the experiment and determine if it could be implemented on a larger scale.

Based on the initial results, researchers hope to pursue additional studies involving children with disabilities and their family dogs, if funding can be secured. They would like to examine other benefits such a pairing might have, including the sense of responsibility the child appears to gain during the course of the intervention.

“We’re also learning a lot from our child participants,” MacDonald said. “They’re teaching us stuff about friendship with the animal and the responsibility of taking care of a pet, which allows us to ask more research questions about the influence of a pet on the child and their family.”

9 out of 10 nurses believe animals can improve patient health

Nine out of ten (90%) nurses believe animals can improve the health of patients with depression and other mental health problems, according to a survey of members of the Royal College of Nursing (RCN).

The survey of more than 750 nursing staff also found that more than 80% think animals can improve communication difficulties such as for people with autism. In addition, 82% said that animals, dogs in particular, encouraged patients to be more physically active, while nearly 60% said just the presence of animals seemed to speed physical recovery.

Nearly half of those surveyed have worked with animals in their career, from dogs and cats to ponies and chipmunks, and of those 98% said it benefited the patient.

However, almost a quarter of staff questioned said no animals were allowed where they worked.

Amanda Cheesley, RCN Professional Lead for Long-term Conditions and End-of-life care, said: “I’ve seen patients with animals in hospitals and in their homes – the difference it makes is remarkable. I used to take my Great Dane with me when I was a district nurse and he could put a smile on any patient’s face.”

“The RCN is calling for better, more consistent access to animals for all patients who can benefit, as the evidence is clear that as well as bringing joyful moments to people when they are unwell, the clinical benefits are tangible. Nurses have told us of patients with reduced anxiety, better interaction and a whole reason to live – and we should listen to these experiences.”

Infection control guidelines for animal visitation

The use of dogs in hospitals and other therapy institutions is on the rise, as more medical professionals acknowledge the positive effects of dogs on human patients.

New expert guidance by the Society for Healthcare Epidemiology of America (SHEA) outlines recommendations for developing policies regarding the use of animals in healthcare facilities, including animal-assisted activities, service animals, research animals and personal pet visitation in acute care hospitals.

The guidance was published online in Infection Control & Hospital Epidemiology, the journal of SHEA.

“Animals have had an increasing presence in healthcare facilities,” said David Weber, MD, MPH, a lead author of the recommendations. “While there may be benefits to patient care, the role of animals in the spread of bacteria is not well understood. We have developed standard infection prevention and control guidance to help protect patients and healthcare providers via animal-to-human transmission in healthcare settings.”

Guidance is grouped by the role of animals – animal-assisted activities (i.e., pet therapy and volunteer programs), service animals, research animals and personal pet visitation. Select recommendations include:

Animal-Assisted Activities

  • Facilities should develop a written policy for animal-assisted activities. An animal-assisted activity visit liaison should be designated.
  • Allow only dogs to serve in animal-assisted activities, such as pet therapy.
  • Animals and handlers should be formally trained and evaluated.
  • Animal interaction areas should be determined in collaboration with the Infection Prevention and Control team and clinical staff should be educated about the program.
  • Animal handlers must have all required immunizations, restrict contact of their animal to patient(s) visited and prevent the animal from having contact with invasive devices, and require that everyone who touches the animal to practice hand hygiene before and after contact.
  • The hospital should maintain a log of all animal-assisted activities visits including rooms and persons visited for potential contact tracing.

Service Animals

  • The policy allowing service animals of patients and visitors into the facility should be compliant with the Federal Americans for Disability Act (ADA), other applicable state and local regulations and include a statement that only dogs and miniature horses are recognized as Service Animals under federal law.
  • If an inpatient has a service animal, notification should be made to the Infection Prevention and Control Team, followed by discussion with the patient to make sure the owner of the service animal complies with institutional policies.
  • Healthcare providers or staff may ask the patient or visitor to describe what work/tasks the dog performs for the patient, but may not ask for a “certification” or “papers.”

Personal Pet Visitation

  • Pets should, in general, be prohibited from entering the healthcare facility.
  • Exceptions can be considered if the healthcare team determines that visitation with a pet would be of benefit to the patient and can be performed with limited risk. Even then, visitation should be restricted to dogs.
  • The patient must perform hand hygiene immediately before and after contact with the animal.

The authors of the guidance also note that as the role of animals in healthcare evolves, there is a need for stronger research to establish evidence-based guidelines to manage the risk to patients and healthcare providers.

This guidance on animals in healthcare facilities has been endorsed by the Association for Professionals in Infection Control and Epidemiology (APIC), the leading professional association for infection preventionists with more than 15,000 members.

Pet therapy and how it helps cancer patients

Therapy dogs may improve the emotional well-being of some cancer patients, according to results of a clinical study, the first to document the benefits of animal-assisted therapy in adult cancer patients. The research was published this month in the Journal of Community and Supportive Oncology.

The results show a significant improvement in quality of life for more than 40 patients who took part in a trial at Mount Sinai Beth Israel in New York City, where they interacted with therapy dogs following chemotherapy and radiation treatments.

The study, conducted by researchers at Mount Sinai Beth Israel, found that patients receiving intensive multi-modal radiation therapy and chemotherapy for gastrointestinal, head or neck cancers experienced increases in emotional well-being and quality of life when they received visits from a certified therapy dog during the course of their treatment.

Increases in emotional well-being were significant over the course of the animal-assisted visits, even as patients underwent marked and significant declines in both physical and functional well-being.

“This study is the first such definitive study in cancer, and it highlights the merits of animal- assisted visits using the same scientific standards as we hold for the cancer treatment itself. It shows the importance of an innovative environmental intervention during cancer treatment,” said Stewart B. Fleishman, MD, principal investigator and Founding Director of Cancer Supportive Services at Mount Sinai Beth Israel. “Having an animal-assisted visit significantly improved their quality of life and ‘humanized’ a high-tech treatment,” he said. “Patients said they would have stopped their treatments before completion, except for the presence of the certified Good Dog Foundation therapy dog and volunteer handler.”

“Thanks to this rigorously designed study, we now have strong evidence that pet therapy is an effective tool to help cancer patients get through challenging treatments,” said Gabriel A. Sara, MD, Medical Director, Infusion Suite at Mount Sinai Roosevelt, and Assistant Clinical Professor of Medicine, Icahn School of Medicine at Mount Sinai.

The study has been called another piece of ‘mounting evidence’ of how the human-animal bond can enhance emotional and physical health.

Animal therapy’s positive effects on college students

Animal-assisted therapy can reduce symptoms of anxiety and loneliness among college students, according to researchers at Georgia State University, Idaho State University and Savannah College of Art and Design. Their findings are published in the latest issue of the Journal of Creativity in Mental Health.

The researchers provided animal-assisted therapy to 55 students in a group setting at a small arts college in the southeastern United States.  They found a 60 percent decrease in self-reported anxiety and loneliness symptoms following animal-assisted therapy, in which a registered therapy dog was under the supervision of a licensed mental health practitioner.

Eighty-four percent of the participants reported their interaction with the therapy dog, Sophie, was the most significant part of the program.

The group sessions were held twice monthly during an academic quarter. Students were invited to stop by and interact with the therapy dog as long as they wished, up to two hours. They were allowed to pet, hug, feed, brush, draw, photograph, sit near and play fetch with the therapy dog.

Dr. Leslie Stewart of Idaho State, who led the study, began the research as a Ph.D. student at Georgia State. She collaborated with Drs. Franco Dispenza, Lindy Parker and Catherine Chang of Georgia State and Ms. Taffey Cunnien of Savannah College of Art and Design.

The prevalence of anxiety and loneliness on college campuses has increased, placing extra demands on college counseling centers. Budget strains have made it necessary for these centers to find creative ways to meet the needs of their students.

This study suggests animal-assisted therapy could be an effective way for college counseling centers to meet the growing demands of their students. It is one of the first to apply animal-assisted therapy in a group, college setting and use a systematic form of measurement.

“College counseling centers are also becoming more and more reflective of community mental health agencies,” Dispenza said. “That’s something that’s been noted in the field in probably the last 10 to 15 years. College counseling centers aren’t seeing students struggling with academics, which major to pick or how to study. They’re coming in with post-traumatic stress disorder, anxiety disorders, pervasive mood disorders and considerable contextual strains that are happening out in the world, such as poverty and experiences of homelessness, as well as a history of medical issues and family health issues.”

To become a registered therapy dog, the animal and handler must complete a series of evaluations and courses, which involve their grooming, temperament, previous training and relationship with their handler. Dogs can be ideal therapy animals because they have become so domesticated and the seeming ability to read cues between dogs and humans is probably the most pronounced. For instance, a dog can tell when a human is sad, Dispenza said.

“The presence of a therapy dog facilitates a therapeutic connection between the client and the mental health professional,” Parker said. “When you’re trying to do mental health work with someone, establishing that therapeutic relationship and rapport is so important. Any way to do it faster or more effectively only helps facilitate the therapeutic process.”

Animal-assisted therapy: less pain medication required

Patients recovering from total joint replacement surgery who receive animal-assisted therapy (AAT) require less pain medication than those who do not experience this type of therapy.  AAT has been used in a variety of healthcare settings to improve quality of life and physical, social, emotional and/or cognitive health for patients.

This retrospective study measured the need for oral pain medication in patients who were exposed to animal-assisted therapy and those who were not. The groups were similar in age, gender, ethnicity, length of stay and type of total joint replacement. The animal-assisted therapy consisted of daily visits from specially trained dogs for an average of five to 15 minutes. The need for oral pain medication was significantly less (28 percent less) in the animal-assisted therapy group (15.32 mg versus 21.16 mg).

This study offers interesting observations about the healing potential of animals,” said Fran Vlasses, PhD, RN, NEA-BC, ANEF, FAAN, co-author and associate professor and chair, Health Systems, Leadership and Policy Department, Loyola University Chicago Marcella Niehoff School of Nursing. “The efficacy of animal-assisted therapy in decreasing the need for pain medication and its effect on patient well-being after surgery deserves further study.”

These data were published in the August/September issue of Anthrozoos by researchers from Loyola University Chicago Marcella Niehoff School of Nursing and Loyola University Health System. Anthrozoos is the official journal of the International Society for Anthrozoology.

Dogs are a ‘social lubricant’ in helping people with autism

Research published in the open access journal PLoS ONE indicates that the presence of an animal can significantly increase positive social behaviors in children with autism spectrum disorders (ASD).

Previous studies have shown that people are more likely to receive overtures of friendship from strangers when walking a dog than when walking alone. The authors suggest that this ‘social lubricant’ effect of animals on human social interactions can be particularly important for individuals with socio-emotional disabilities.

In this study, the authors compared how 5-13 year old children with ASD interacted with adults and typically-developing peers in the presence of two guinea pigs compared to toys.  When the guinea pigs were present, the children were more likely to talk and look other people in the eye plus they smiled and laughed more often.

These results indicate that animal-assisted therapy programmes may be useful in helping children with ASD integrate into classrooms and other social environments.
