A vaccine for canine osteosarcoma?

Osteosarcoma is a highly aggressive bone tumor that affects at least 10,000 dogs annually in the United States, alone.

It is estimated that 90-95 percent of canine osteosarcoma subjects have microscopic metastatic disease (spread of cancer cells to other parts of the body at the time of diagnosis). Standard of care includes removal of the primary tumor—usually by amputation—followed by chemotherapy. Systemic chemotherapy given after amputation delays the development of metastatic disease; however, despite treatment, most dogs die of the disease within one year of diagnosis.

A new option may be available in the future if Dr. Nicola Mason’s research at the University of Pennsylvania School of Veterinary Medicine produces satisfactory results. Mason, an assistant professor of medicine and a Board-certified ACVIM Diplomate in Small Animal Internal Medicine, is evaluating the first vaccine for canine osteosarcoma.

The approach harnesses the power of the dog’s immune system, “training” it to seek out and destroy cancer cells that remain after amputation and chemotherapy.

Over a century ago, an orthopedic surgeon named William Coley recognized that human sarcoma patients with concurrent bacterial infections that caused high fevers had improved overall survival times compared to those sarcoma patients without infection. This led him to develop a therapeutic concoction of live bacteria that he injected into patients with bone sarcomas. He documented improved survival and in some cases, complete remission in individuals with the aggressive disease by using this early form of “immune therapy.”

Mason’s team is employing similar immune therapeutic strategy to treat dogs with osteosarcoma that have undergone the standard of care treatment (amputation and chemotherapy) to prevent metastatic disease. “The concept is that administration of the Listeria-based (genetically modified bacteria) vaccine will activate the patient’s immune system and educate it to recognize cells that express the target molecule,” says Mason.

Dogs are given the live bacterial vaccine intravenously, Mason explains, and it induces a mild transient fever on the day of vaccination. The dogs are usually treated as outpatients and return home the same day. “We have found highly encouraging results when the vaccine is given to patients that have no evidence of metastatic disease at the time of the study enrollment, which is three weeks after the last chemotherapy is administered. Four out of the first five dogs vaccinated are alive at least two years after their initial diagnosis, which is more than twice their expected survival duration. The vaccine has not yet shown any serious short- or long-term side effects, either.”

Mason says the results have led researchers to evaluate whether this vaccine may be able to directly target and kill the bone tumor itself, perhaps eliminating the need for amputation in the future.

A dog cancer that is 11,000 years old

Scientists have sequenced the genome of the world’s oldest continuously surviving cancer, a transmissible genital cancer that affects dogs.

The cancer, which causes grotesque genital tumours in dogs around the world, first arose in a single dog that lived about 11,000 years ago. The cancer survived after the death of this dog by the transfer of its cancer cells to other dogs during mating.

Analysis of the genetic variants in the genome revealed that the dog may have resembled an Alaskan Malamute or Husky. It probably had a short, straight coat that was coloured either grey/brown or black. Its genetic sequence could not determine if this dog was a male or a female, but did indicate that it was a relatively inbred individual.

The genome has over 2 million mutations; the research team used one type of mutation, known to accumulate steadily over time, as a ‘molecular clock.’  This led to the estimate that the cancer first arose 11,000 years ago.

“The genome of this remarkable long-lived cancer has demonstrated that, given the right conditions, cancers can continue to survive for more than 10,000 years despite the accumulation of millions of mutations”, says Dr Elizabeth Murchison, first author from the Wellcome Trust Sanger Institute and the University of Cambridge.

Transmissible cancers are very rare in nature.  Apart from the dog transmissible cancer, the only other known naturally occurring transmissible cancer is an aggressive transmissible facial cancer in Tasmanian devils that is spread by biting.

Breast cancer in dogs

A PhD project at the Norwegian School of Veterinary Science has been studying the genetics behind mammary tumours in dogs.  Breast cancer is the most common form of cancer in female dogs.

Kaja Sverdrup Borge’s PhD project has led to the identification of genetic changes associated with these types of tumour.

Borge studied known risk genes in dogs to learn more about the genes that predispose dogs to canine mammary tumours. These genes are already known to be linked to cancer in humans. Borge discovered that there was a large variation in these genes from breed to breed. Some of the variants proved to have a detrimental effect and could lead to a potential change in the risk of developing cancer.

Borge compared the incidence of these genes in different groups of English springer spaniels with and without mammary tumours. The genes were also studied in another group of dogs from breeds having either a high or low incidence of mammary tumours. The results of these analyses indicate that variants of the oestrogen receptor gene are associated with the risk of English springer spanieldeveloping mammary tumours in dogs.

Borge examined canine mammary tumours in order to identify mutations which have arisen in tumours and may therefore be involved in the development of cancer. She focused on changes in the number of gene copies where there was either a decrease or increase in gene areas in the tumours.

She found a large number of mutations and the number of aberrations increased, the more malignant the tumours turned out to be. She detected major cancer genes known to occur in humans but also identified new areas. Borge also demonstrated that linking detailed histopathological parameters from mammary tumour diagnostics to genetic mutations could help to chart specific genes that lead to the growth of tumours and to more malignant types of cancer.

Increased knowledge about the genetic changes which cause cancer is essential for the prevention, diagnosis and treatment of the disease.  Because of the similarities between carcinogenic gene mutations in both canine and human breast cancer, studying breast cancer in dogs also has benefits for the study of breast cancer in humans.

When something isn’t right with your senior dog

Old dog_dementiaWe love them to bits.  And gradually we notice changes that signal they are getting older.  They may no longer hear the doorbell and, thanks to this new deafness, they may sleep very deeply.

But  changes in an old dog need to be considered carefully.  Behavioral changes can often be the signs of other problems, like diabetes, hypothyroidism or hyperthyroidism, dental disease, and cancers.

One thing that I’m learning more about is canine cognitive dysfunction syndrome (CDS).  Veterinarians describe this as a ‘diagnosis of exclusion’ which means they look to diagnose another disease or disorder first before deciding that the dog is suffering from CDS.

When assessing for symptoms and severity of CDS, veterinarians follow the acronym DISHA.  DISHA stands for:

D= Disorientation

I = Interaction changes

S = Sleep/wake cycle changes

H = House soiling

A = Activity level changes

Disorientation can present as changes in spatial awareness, loss of ability to navigate around familiar obstacles, and/or wandering behavior.

Interaction changes can include a decreased interest in social interaction, petting, greetings, or dependent behaviors.

Restlessness or frequent waking during the night, panic or panting (particularly at night), and increased sleep during the daytime are indications of changes to sleep/wake cycles.

House soiling can increase when there is a loss of signal from the brain so your dog doesn’t realise it needs to eliminate; signs of incontinence or fouling indoors when this has never been a problem are symptoms.

Changes in activity can include decreased exploration and response to stimuli, decreased grooming, change in appetite, increased anxiety with signs of restlessness or agitation and/or separation anxiety.

Most vets offer senior wellness checks  for older dogs.  It’s well worth observing your older pet and discussing all changes with your vet before dismissing the changes as simply old age.

Lymphomas in humans and dogs have similarities

A team of scientists from the University of North Carolina School of Medicine, North Carolina State University’s College of Veterinary Medicine and Duke University have conducted one of the first studies to directly compare canine and human B-cell lymphoma by examining molecular similarities and differences between the two species.

B-cell lymphomas are very common in both humans and dogs.

“Pet dogs get cancer the same way humans do: at similar rates, and for unknown reasons.  Like humans, dogs’ tumors are spontaneously occurring, rather than genetically created as they are in mice, so canine tumors may more accurately mimic the situation in human cancer patients. Dogs are good models to study, because it will also be possible to study shared risk factors, in the environment, for example, that might predispose both humans and dogs to get lymphoma. Our knowledge helps dogs and humans with lymphoma.” says Kristy Richards, MD, PhD, and co-author of the research which has been published in the journal Cancer Research.

“Veterinarians treating dogs for lymphoma can offer clinical trials to their owners. Clinical trials in dogs are similar to those done in humans, with safety protections in place to minimize harm.”

Molecular analyses of canine and human tumors were completed at NCSU and at UNC Lineberger. The team used gene expression profiling and found that canine B-cell lymphoma expression profiles were similar in many ways to human B-cell lymphoma, thus paving the way for future studies, including therapeutic clinical trials in dogs and humans.

Canine cancer: the warning signs and the way to a cure?

According to the Animal Cancer Foundation, one in four dogs will develop cancer in their lifetime.  That’s a pretty scary statistic.  A diagnosis of a malignant cancer for one’s dog is just as traumatic and worrying as a diagnosis of the disease in any member of the family.

Experts in both human and canine cancers agree that we still have much to learn about the types of these diseases and how to treat them.

The National Veterinary Cancer Registry has been launched recently to identify and register pets diagnosed with cancer.  The Registry’s goal is to facilitate and promote medical treatments that lead to advances, higher success rates and eventual cures for cancer in pets and people by matching animals with cancer to clinical trials for new cancer treatments.

The Registry is a joint venture between the CARE Foundation, Baylor University Medical Center (BUMC) at Dallas and the Texas Veterinary Oncology Group.

Dr Gerald S Post, Founder and President of the Animal Cancer Foundation says there are 10 warning signs of cancer in both dogs and cats.  They are:

1. Swollen lymph nodes: These “glands” are located all throughout the body but are most easily detected behind the jaw or behind the knee. When these lymph nodes are enlarged they can suggest a common form of cancer called lymphoma. A biopsy or cytology of these enlarged lymph nodes can aid in the diagnosis.

2. An enlarging or changing lump: Any lump on a pet that is rapidly growing or changing in texture or shape should have a biopsy. Lumps belong in biopsy jars, not on pets.

3. Abdominal distension: When the “stomach” or belly becomes rapidly enlarged, this may suggest a mass or tumor in the abdomen or it may indicate some bleeding that is occurring in this area. A radiograph or an ultrasound of the abdomen can be very useful.

4. Chronic weight loss: When a pet is losing weight and you have not put your pet on a diet, you should have your pet checked. This sign is not diagnostic for cancer, but can indicate that something is wrong. Many cancer patients have weight loss.

5. Chronic vomiting or diarrhea: Unexplained vomiting or diarrhea should prompt further investigation. Often tumors of the gastrointestinal tract can cause chronic vomiting and/or diarrhea. Radiographs, ultrasound examinations and endoscopy are useful diagnostic tools when this occurs.

6. Unexplained bleeding: Bleeding from the mouth, nose, penis, vagina or gums that is not due to trauma should be examined. Although bleeding disorders do occur in pets, they usually are discovered while pets are young. If unexplained bleeding starts when a pet is old, a thorough search should be undertaken.

7. Cough: A dry, non-productive cough in an older pet should prompt chest radiographs to be taken. This type of cough is the most common sign of lung cancer. Please remember there are many causes of cough in dogs and cats.

8. Lameness: Unexplained lameness especially in large or giant breed dogs is a very common sign of bone cancer. Radiographs of the affected area are useful for detecting cancer of the bone.

9. Straining to urinate: Straining to urinate and blood in the urine usually indicate a common urinary tract infection; if the straining and bleeding are not rapidly controlled with antibiotics or are recurrent, cancer of the bladder may be the underlying cause. Cystoscopy or other techniques that allow a veterinarian to take a biopsy of the bladder are useful and sometimes necessary to establish a definitive diagnosis in these cases.

10. Oral odor: Oral tumors do occur in pets and can cause a pet to change its food preference (i.e. from hard to soft foods) or cause a pet to change the manner in which it chews its food. Many times a foul odor can be detected in pets with oral tumors. A thorough oral examination with radiographs or CT scan, necessitating sedation, is often necessary to determine the cause of the problem.

Helping pet owners make tough choices

When your dog becomes seriously ill, it’s your job as the owner to make decisions about quality of life.  And it’s one of the toughest decisions we face during our lives.

Researchers at Michigan State University  are developing a new tool to help people assess their pet’s quality of life, a key factor in decisions about when to order life-prolonging procedures and when an animal’s suffering means it’s time to put them to sleep.

The research team, led by veterinarian Maria Iliopoulou, created a survey to help dog owners monitor the quality of life of 29 dogs undergoing chemotherapy treatment for cancer.   The owners completed the questionnaire when they received their dog’s cancer diagnosis and answered questions about how their dog was behaving then and how they behaved six months prior to the diagnosis.

Similar questions were asked in questionnaires administered at three and six weeks into chemotherapy. Meanwhile, the veterinarians treating the dogs filled out shorter surveys based on their observations.   The research team wanted to see if owners and clinicians agreed.

The research found that there was a close match between owners and vets, particularly in questions involving play behaviour, the dog’s happiness as perceived by the owner and clinical signs of disease.  These areas of commonality create the basis for a tool that will help to facilitate client and vet communication.  If there’s agreement about what constitutes quality of life, then it is these criteria that owners and vets should use to help agree on next steps for the dog’s care.

For the study, dog owners completed a questionnaire at the time of diagnosis about how the animal was behaving then and how they typically behaved six months prior. Follow-up questionnaires filled out three and six weeks later documented changes in behavior as the dogs underwent chemo. The research team has published their results in the Journal of the American Veterinary Medical Association.  All dogs were patients at the Michigan State University Animal Cancer Care Clinic.  The plan is to expand the work using a much larger sample size of patients and Iliopoulou hopes to develop questionnaires for dogs suffering from other diseases as well.

Pukka’s Promise

Pukka's Promise coverIf I had to choose a byline for this book review, it would be ‘Ted Does It Again.”

Author Ted Kerasote has delivered another great dog book following the success of Merle’s Door which I have previously reviewed.

This book, inspired in part by the large volume of correspondence Ted received after releasing Merle’s story, documents Ted’s extensive research into the health of dogs and the factors that may determine longevity.   So many ‘dog people’ contacted Kerasote asking variations of the same question  – ‘why don’t our dogs live longer?’  And since Ted felt the same way, he did what any professional journalist would do – he asked lots of questions.

In Pukka’s Promise -The Quest for Longer-Lived Dogs, Ted tackles subjects including nutrition, the politics behind the pet food industry, and what diet is right for dogs.  Ted lays out the facts about raw food and commercial diets, the concerns about grains  and whether they are appropriate for dogs and a favourite topic of mine – variety in the diet.

Because Kerasote observes his dog so well, he realises that there are times when Pukka (pronounced PUCK-ah) rejects the food that is laid before him.  Pukka lets Ted know that he prefers something else one day when he follows him into the pantry.  Having rejected raw lamb, Pukka readily accepts some dried elk chips.  Ted then listens more often to what Pukka would like for his meals noting, “Today I do not want sardines, I want chicken.  Yes, I do love elk, but this evening I prefer dried elk.”

And just as he did in Merle’s Door, this communication between Kerasote and Pukka is not contrived nor do these moments come across as a story book type of anthropomorphism.  Kerasote is a keen observer and dog aficionado.  When he listens or hears Pukka, it’s because he understands what his dog is trying to tell him and translates it into words.  Few authors could achieve this in such a natural way.

An example of the communication between Ted and Pukka comes when Ted is frustrated by Pukka’s excessive barking.  Dog trainers should be prepared that Ted’s solution doesn’t come from clicker training or positive reinforcement, although Ted tries these things.  Ted’s solution is a direct result of understanding dog behaviour and putting that knowledge to good use.  It helps that Ted can communicate in dog.   Enough said; you’ll have to read the book for the ending of this tale.

Kerasote covers a range of health topics including vaccinations, the history of the ‘annual vaccination’ recommendation, and the latest research on why over-vaccinating is a concern.  A good message to take away from reading the book is to enquire with your vet about having your dog ‘titered’ to determine the amount of immunity they still have from previous vaccinations.

Still other issues that are tackled in a thorough way are the effects of neutering and alternatives to the traditional spay/neuter operation that may help our dogs retain the health-preserving effects of their natural sex hormones.  Kerasote also questions the spay/neuter philosophy in a constructive way and whether you agree with his conclusions or not, he does lay out the facts very well.

Another topic that I hold dear is the issue of cancer and the simple message – if you find a lump on your dog, don’t let anyone (including your vet) tell you to ‘wait and see.’  Some lumps, if caught early and tested, can be removed before the disease takes over the comparatively small body of a dog.  Take heed!

As a backdrop to the book’s hard facts, we also get to enjoy a wonderful story about Ted’s search for another dog and his choice of Pukka.  Once Pukka’s is on the scene, we share some of their adventures.

My only criticism of this book is its lack of photos.  Other than the cover photo of Pukka, we don’t get to enjoy any photos of Pukka, Ted, or their other dog friends (A.J., Burley and Goo) nor any of the great scenery from Ted’s camping and hunting trips with Pukka.  I don’t think photos would have detracted from the contents and scope of the book, but I guess that’s the publisher’s decision.

With 49 pages of references, this is a thoroughly researched book that took five years to complete.  Add it to your book collection and refer back to it as the basis for a conversation with your vet (your dog will love you for it).

How Chinese medicine may help dogs with cancer

Research from the University of Pennsylvania School of Veterinary Medicine has been published which shows promise in treating dogs with a specific type of cancer.

Hemangiosarcoma is a type of cancer that can occur in any dog, but German Shepherds and Golden Retrievers seem to be the most prone to developing it.  The cancer is an aggressive and malignant tumor of blood vessel cells.   The most common area it affects is the spleen but can appear in the skin, other soft tissues and the liver.  This is a type of cancer that is metastatic and it will frequently spread to other parts of the body.

In the study, 15 dogs were divided into three groups of 5 and given a different dose of a product called I’m-Yunity, which contains polysaccharopeptide or PSP.  PSP is derived from Yunzhi mushrooms and appears to have an immune-boosting effect – it’s been used in traditional Chinese medicine for more than 2,000 years.

The findings?

‘We were shocked,’ says Professor Dorothy Cimino-Brown.  ‘Prior to this, the longest reported median survival time of dogs with hemangiosarcoma of the spleen that underwent no further treatment was 86 days.  We had dogs that lived beyond a year with nothing other than this mushroom as treatment.’

The treatment may offer a way for owners to extend the life of their pet and there were no adverse effects reported.

The researchers plan to study the effect of the compound further, using groups of candidates that are given a placebo, others which undergo chemotherapy, and other groups given the mushroom-based treatment.
